2. Performance ratings for the period July-December 2022 (if applicable). Numerical and equivalent rating must be indicated;
3. Authenticated photocopy of Certificate of Eligibility issued by the CSC/valid Professional License issued by the PRC;
4. Authenticated photocopy of Transcript of Records issued by the University Registrar;
5. Certificate of attendance to trainings, seminars and workshops for the last five (5) years (June 2018 - May 2023);
6. Certificate of employment from previous employer(s) for work experience relevant to the job;
7. Certificate of completion of research conducted and copy of peer-reviewed journals.
