Education: High School Graduate
Training: Four (4) hours of relevant training
Experience: One (1) year of relevant experience
Eligibility: Security Guard License (MC No. 10, s. 2013 Category IV)
Place of Assignment: Security Service Force, UPV Miagao campus

1. Application letter. Please specify the position title and plantilla item number being applied;
2. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) which can be downloaded at csc.gov.ph;
3. Fully accomplished Work Experience Sheet (WES), attachment to CS Form No. 212, which can be downloaded at www.csc.gov.ph;
4. Certificate of Professional Eligibility issued by the Civil Service Commission;
5. Diploma and Official Transcript of Records issued by the University Registrar;
6. Certificate of employment issued by HRMO or authorized official (only for work experience relevant to the job);
7. Certificate of performance rating obtained in the last rating period of current or last employment or Certification of GWA for fresh graduates;
8. Photocopy or authenticated copy of Certificate of attendance to relevant formal and non-formal training courses, seminars and HRD interventions attended or from the point of the last promotion;
9. Certification of Awards/Citations/Commendation (has not been credited for last promotion) and Innovations/Systems Improvements.
10. Certification of public service rendered (training program, technical assistance and advisory, information services, community outreach services, innovations and technology transfer services and direct extension services).
