2. Photocopy of Certification of Highest Educational Attainment issued by the Department of Education / Transcript of Records and Diploma issued by the University Registar;
3. Photocopy of Valid Professional Driver's License issued by the Land Transportation Office;
4. Certificate of employment issued by the HRMO or authorized official (only for work experience relevant to the job);
5. Certificate of performance ratings obtained in the last two (2) rating periods (January to June 2021 and July to December 2021);
6. Certificate of attendance to trainings, seminars and workshops for the last five (5) years (May 2017 to May 2022);
7. Certificate of Accomplishments (Awards/Citations/Innovations/Critical Incidents)
